STUDENT WAIVER & RELEASE OF LIABILITY I, the Participant or Parent/Guardian, desire to take part in Swim Lessons (hereinafter referred to as the "Activity") and/or other aquatic programs conducted with Mr. Jordan Swims. I am aware that engaging in these Activities carries inherent risks, including the potential for physical harm, loss of life, or damage to property. I acknowledge and consent that I/my child have/has chosen to participate willingly, understanding the associated risks. I confirm that neither I nor my child have any known physical or medical conditions that would impede our ability to take part. I hereby waive, release, and forever discharge any and all claims for damages for personal injury, loss of life, or property damage which I may have, or which may hereafter inure to me, as a result of participation in the Activity. I hereby acknowledge that participation in the Activity does not guaranty that I/my child will be water-safe. This release is intended to discharge in advance Mr. Jordan Swims or any Mr. Jordan Swims Subsidiaries along with all of their employees, volunteers, officers, and representatives from any and all liability arising out of, or connected in any way, with my participation in the Activity. I agree to indemnify and to hold Mr. Jordan Swims and any Mr. Jordan Swims Subsidiaries, as well as employees, volunteers, officers, and representatives free and harmless from any loss, liability, damage, cost, or expense which they may incur as the result of my or my child's loss of life or injury or damage that I may sustain while participating in the Activity. REGISTRATION & REFUND POLICY By signing up for a lesson, I understand and acknowledge that all sales are final and non-refundable. No refunds will be issued after registration, regardless of circumstance. I understand that lesson spots are limited and that by claiming a spot I am removing it from availability for others. If I am unable to attend my scheduled lesson, Mr. Jordan will make every reasonable effort to accommodate me by moving my spot to a different group lesson session. If I need to cancel entirely, I have the option to sell or transfer my spot to another individual. Mr. Jordan Swims is not responsible for facilitating such a transfer. Parental Consent (Must be completed and signed by Parent if participant is under 18 years of age) I hereby consent that [your child's name] may participate in the Activity, and I hereby execute the above agreement, waiver, and release on his/her behalf. I hereby assert that my child is physically able to participate in said activity. I hereby agree to indemnify and hold harmless the Instructor from any loss, liability, damage, cost, or expense, which may occur as a result of loss of life or injury, or property damage, that my child may sustain, while participating in the Activity. I HAVE CAREFULLY READ THIS AGREEMENT, WAIVER, AND RELEASE AND FULLY UNDERSTAND ITS CONTENT.
